Practice Policies & Membership Guidelines

Our Commitment to Quality Care

To maintain the high standard of Functional Medicine and personalized attention Dr. Nicole provides, Perfect Touch Massage & Chiropractic operates on a structured membership and scheduling system. By limiting our patient roster, we ensure that you never feel like a number and always have the time required for a root-cause analysis. Our policies reflect this.

Appointment & Cancellation Policies

Your time is valuable, and so is ours. Because our appointments are significantly longer than standard “in-and-out” clinics, a late cancellation affects our ability to serve other members.

  • 24-Hour Notice: We require at least 24 hours’ notice for all appointment cancellations or rescheduling.
  • Late Cancellations: Appointments cancelled with less than 24 hours’ notice may be subject to a cancellation fee.
  • No-Shows: Consistent no-shows may result in a required review of your membership status to ensure our limited spots are available for active patients. *For medical massage cancellations, there is a no-show fee of half the price if you do not cancel before the 24 hour window.

Membership Terms & Billing Policies

Our Concierge Wellness Memberships are designed for long-term health transformation.

  • Billing Cycle: Memberships are billed monthly on your signup anniversary date.
  • Renewal: Memberships are annualized and will renew unless there is a 30-day written notice of change, or cancelation.
  • Cancellation: Memberships are an annual contract. They can be cancelled with a 30-day written notice prior to the renewal date of the annual membership.

Privacy and HIPAA Compliance

Your health data is your most sensitive information. We utilize secure, encrypted platforms (like Practice Better) for all messaging, lab results, and genetic data sharing. We strictly adhere to HIPAA guidelines to ensure your privacy is never compromised.

Insurance Policies

In order to server our patients in the most cost effective way, and be able to care for you with any and all ways necessary to get you better and back to health, we do not take health insurance as a form of payment in our office.

Can I use my HSA/FSA for my membership?  

Yes! Most of our functional medicine and chiropractic services are HSA/FSA eligible. We provide itemized receipts (Superbills) upon request to help you maximize your benefits.

We accept: HSA’s, Flex Spending, cash, and all major credit cards. We apologize for any inconvenience this may cause you. For those of you that want to do your own submitting for reimbursement, we have super bills that are directly uploaded on your patient portal that you will be able to submit to the appropriate insurances on your own. Make sure you contact them directly to ensure you can submit for reimbursement.

Check Policies

We do not accept checks until you are a client/patient for (6) visits. No exception. We do however accept all major credit cards: Visa, MasterCard, Discover and American Express.

Refund Polocies

All credit card and/or debit card refunds will be the original transaction amount less the services used and payment processing fees. All cash and/or check refunds will issued as a check for the original transaction amount less any services used.

Credit Card Fee

Due to the increase in fees in our credit card companies and the other fees tacked on to our business. We are choosing to charge a 3% credit or debit card fee when paying your invoice or service in office effective 1/28/25. This makes it so we don’t have to raise our fees for service. Thank you for understanding.

How to avoid the fees: pay with cash in office or if you have been with us for awhile you can also opt to pay with a check.

Returned Item Fee

There will be a $30 Non-Sufficient Funds fee if your credit card, debit card, or check is sent back to us as a returned item from your financial institution.

Lab Testing

No tests will be ordered for any person who is not under the direct care and supervision of Dr. Nicole Klersy-Mohr. Lab tests are one of the many tools Dr. Nicole utilizes to maintain the best possible care for her patients. They are only ordered for active patients whom are currently following treatment plans designed specifically for them by Dr. Nicole.
